HYBRID CARE

Hybrid Care

Hybrid Care is a great option for people who have a high-risk pregnancy or who otherwise ultimately feel most comfortable birthing in a hospital. This integrative model allows you to be held within the midwifery model of care while receiving concurrent care from your hospital provider.

Prenatal care

What to expect:
Your midwives provide prenatal care in addition to visits you have with your hospital provider. Prenatal appointments are an hour long. You will have visits with the midwife every 4 weeks until 28 weeks, every 2 weeks from 28-36 weeks, and every week from 36 weeks until your baby is born.

● In-home blood draws and cultures for indicated blood work including any of the following
tests: initial pregnancy panel, Gestational Diabetes testing, GBS testing, NIPT genetic
testing.
● Nutrition and lifestyle recommendations from a holistic perspective.
● Comprehensive health assessment of you and your babe at every visit including; hands
on growth estimation, fetal position monitoring/checking, fetal heart tone monitoring and
an overall wellbeing assessment for baby. We will also perform blood pressure
monitoring, pulse rate, and wellness checks for the pregnant person.
● Prenatal, birth, and postpartum education and referrals.
● Psychosocial and emotional check-ins to hold space for all the shifts that are part of
pregnancy, birth and postpartum.
● Support in developing a birth plan.
● Discussion about what to pack in your hospital bag.

Labor and birth

Labor will begin in your home and one of our midwives will join you in active labor. The wellbeing of both you and your babe will be monitored while at home. With the help of your midwife, you will decide when to transition to the hospital. Your midwife assistant / doula will accompany you to the hospital and advocate for you and your chosen birth-plan.
What to expect:
  • Intermittent fetal monitoring and assessment
monitoring of all vitals including blood pressure, pulse, temperature, fluid input and output, and comfort levels.
  • Strategizing around rest, nourishment, and hydration.
  • Cervical dilation assessment, if desired.
  • Collective decision making on when to go to the hospital.
  • Emotional, educational, and physical support for you in the hospital under the clinical care of your hospital provider.
  • 1-2 hours of hands-on support after your babe is born to help with transitioning, latching, settling in, and bonding.
  • The option for an early discharge where our midwives will resume clinical care for you and your little one at home

Postpartum care

Once you are discharged from the hospital, our midwives will resume postpartum support for you and your babe in the comfort of your home. Depending on the discharge day and time, I will make home visits on day 1, day 3, around 1 week, 2 weeks. The 4 week, and 6 weeks postpartum are back at my home office.
What to expect:
  • An assessment of vitals and wellness for both you and babe
  • Weight checks for baby
  • Lactation and breast/chestfeeding support
  • Monitoring of your bleeding and your uterine health
  • In the case of an early discharge – an administration of the Newborn Screen
  • Visual jaundice screening
  • Education suggestions surrounding postpartum movement, nutrition, sleep and emotional wellness
  • Referrals to other postpartum care providers and services
